آپ  یا آپ کے عزیز و اقارب میں سے کوئی کسی ایسی تکلیف میں مبتلا ہے کہ باوجود مختلف علاج معالجے  کےخلاصی کی کوئی راہ نہیں پارہےتو ہمارے دارے کی  خدمات ضرورحاصل کریں۔ ایک ماہ میڈیسن استعمال کریں، اگر مریض بہتری محسوس کرے اور رپورٹس بھی بہتر ہوجائیں تو باقی علاج بھی اعتماد کے ساتھ مکمل کرلیں۔

پرانے اور پیچیدہ امراض کے علاج کے لئے کیس ریکارڈ فارم یہاں سے ڈاؤن لوڈ کیجئے۔ کیس ریکارڈ فارم کو پُر کرنے سے پہلے اچھی طرح اس کا مطالعہ کیجئے تاکہ آپ کو ہمارے علاج کا طریقہ سمجھنے میں آسانی ہوسکے۔ کیس ریکارڈ فارم ڈاؤن لاوڈ کرنے کے بعد اس کا پرنٹ نکلوائیں اور سوالات کے جوابات لکھیں۔ صرف انہی سوالات کے جوا ب لکھیں جو آپ کے مرض کے متعلقہ ہیں۔ ہرسوال کا جواب لکھنا ضروری نہیں۔   فارم فل کرنے کے بعد اس پر لکھے ہوئے ایڈریس پر کوریئر کردیں۔ فارم وصول ہونے کے بعد آپ کو اپائنٹمنٹ دے دی جائے گی۔ فارم یہاں سے ڈاؤن لوڈ کیجئے
